Meyer and Dana Orthodontics is proud to announce the new Black Hills Chapter of Smile for a Lifetime! Smile for a Lifetime Foundation is a non-profit, charitable organization comprised of local chapters throughout the country that provides quality orthodontic treatment for patients who may not have the opportunity to achieve a beautiful, straight smile without financial assistance.

At Smile for a Lifetime Foundation, it is our mission to create self-confidence, inspire hope, and change the lives of children in our communities in a dramatic way. The gift of a smile can do all this for a deserving, underserved individual who, in turn, can use this gift to better themselves and their community.

Please note that there are several documents that must accompany the application. Along with the completed application, please submit

• A 5x7 head shot with full smile and teeth showing
• Two letters of reference, typed and limited to one page each
  
• Verification of family income, which can be last year's tax return
  
• A current W-2 or a copy of a recent paystub

All applications, pictures and supporting documents will not be returned and become property of Smile for a Lifetime Foundation.
